2017 Home Energy Assistance Program (HEAP) Summer Crisis Program Now Available!

HEAP starts today and runs until August 31st, 2017. 
HEAP offers assistance with electric utility bills.  Limited supply of air conditioners may be available, based on eligibility.

Requirements for HEAP:
  • Income Eligible
  • Households must have an individual either:  60 years or older or have a medical condition certified by a physician 
  • Must not be a current PIPP Plus Customer for electric assistance
  • For air conditioners, must not have received an air conditioner in the last 3 years.

PRC through DJFS (327-1700) can help with various emergency financial assistance, you must have an income and children to qualify.

Mobile Mammography Schedule

Mobile Mammography ... because life is busy

CMHP provides Mobile Mammography services for easier access to quality care. The mobile unit uses the same high-quality technology and equipment you will find at the Springfield Regional Imaging Center with the flexibility of 3D and 2D screening options.
The Mobile Mammography program provides convenient 15-minute screening appointment. No doctor order is needed to make an appointment.

How to bring the Mobile Mammography unit to you?
The mobile unit is available at no charge to employers and organizations who would like to offer these health services for their employees and community. Please call our Coordinator at 937-523-9330 for more information on sponsoring mobile mammography and to reserve your event date.
Why do I need a mammogram?
A mammogram is the best screening tool for early detection of breast cancer while chances of survival are highest. One in eight women will develop breast cancer in her lifetime. 85% of those women have no family history. Symptoms of breast cancer are not the same in all women. It is recommended all women get a mammogram each year starting at age 40 with a baseline screening age between 35 and 39.

To schedule your 3D or 2D mammogram, call 937-523-9332. Appointments are approximately 15 minutes if pre-registered. No doctor order needed for screening.  Mammograms are covered by most health insurance plans. Check with your insurance provider for exact coverage. No-cost screenings available for those who qualify. When you call to register, ask about our financial assistance options.  Be sure to bring a photo ID and insurance card with you.

Income Tax Preparation Assistance 2017


FREE income tax preparation.  Please bring: Photo ID’s and SS cards for everyone in household and last year’s tax return, W2’s, unemployment income, 1099s, homeowner’s mortgage interest and real estate taxes paid,  information on health insurance coverage for everyone in the household.
